Lucian Leape, an adjunct professor of health policy at the Harvard School of Public Health, conducted the most thorough examination of medical errors in the United States to date. “It’s by far the number one problem,” said Leape.
Leape and other experts say that typically more than 90% of medical errors are hidden not only from the public, but from hospital authorities as well. They estimate that documented mistakes account for about 5 to 10% of the total number of mistakes made at a typical hospital. The rest are covered up.

According to Dr. Vinay Prasad, MD, of the Medical Oncology Branch at the National Cancer Institute at the National Institutes of Health in Bethesda, MD, 40% of current medical practices offer no benefits. Dr. Prasad and his team evaluated 1,344 original articles published in the New England Journal of Medicine between 2001 and 2010 and identified 146 medical reversals. Medical reversals refer to medical practices that offer no benefits and have never worked. Says Dr. Prasad: “They were instituted in error, never helped patients, and have eroded trust in medicine.” |
False Cancer Diagnoses and Unnecessary Chemotherapy
Most of the time, the mistakes doctors make—even if they cover them up—are truly accidents. But not always. U.S. Attorney Barbara McQuade is charging the popular physician, who at one time had more than a thousand patients under his care, with healthcare fraud.
Prosecutors say Fata, a trusted oncologist, “preyed on patients to pad his pockets. His alleged wrongdoings include:
Administering expensive and uncalled-for testing and treatments
Giving patients “unnecessary chemotherapy,” even for “end-of-life patients who will not benefit”
“Deliberate misdiagnosis,” including falsely diagnosing patients with cancer
Court records clearly show current and former employees, concerned by the magnitude of Dr. Fata’s malpractice, turned into FBI informants. According to one such informant, working for Fata came with the heavy burden of “living with this hell.”
Dr. Fata’s patients are shocked and afraid. After hearing about the charges Fata faced, Jeff Berz reached out to federal prosecutors. He fears his father Milton, who saw Fata for treatment for his leukemia, suffered through unnecessary chemotherapy. As Milton’s health declined, Fata continued to push chemotherapy, and took extreme measures to distribute it.
In the wake of these shocking revelations of wrongdoing, experts are making the expected recommendations, for instance: always be alert at the doctor’s… request and read copies of your medical records… and get a second opinion, especially for serious diagnoses.
But why should patients have to jump through hoops to receive proper care? History proves even well-liked and trusted professionals (like Fata) succumb to the tempting lure of a big payoff for falsifying information and administering unnecessary treatments.
